Sample Self-Assessment: Crisis Management and Incident Response

This is a sample of one category from the PreparedEx Resilience Program Maturity Assessment. The full assessment covers ten categories; this walkthrough covers Category 4. Crisis Management and Incident Response only.

For each of the ten questions you will:

  1. Indicate the evidence status: whether documented evidence for the practice exists (Provided), exists in part (Partially Provided), or does not exist (Not Provided).
  2. Score yourself from 0 to 5 using the maturity scale below, or select NA if the question does not apply.
  3. Optionally add notes to capture context or gaps.

Your category score is the sum of your ten question scores (maximum 50) and is mapped to a maturity level at the end.

0Nonexistent: no capability in place, and no efforts have been made in this area.
1Initial: minimal or ad hoc capability; some preliminary actions may exist, but they are unstructured and lack formal processes.
2Developing: basic capability established with some structured actions, though gaps remain in consistency and effectiveness.
3Defined: standard practices are in place; they are somewhat effective but not consistently applied or integrated across the organization.
4Managed: mature and effective capability with formal processes; consistently applied and well-integrated, with some ongoing improvement efforts.
5Optimized: best-in-class capability with fully integrated, continuously improved processes; resilience is deeply embedded in the organizational culture.
